This Membership Agreement (“Membership”) is between ZenXtreme and the participant (“Participant”).
By submitting your Membership order with valid payment information, you agree to our Membership Terms.
I, _________ (Participant), authorize ZenXtreme to charge my payment method for scheduled fees, drop-in classes, workshops, and other account balances.
This is a periodic charge starting on the purchase date, which is billed monthly for the indicated amount. I may terminate billing anytime with 30 Days’ notice on ZenXtreme’s My Account page.
Cancellations must be made at zenxtreme.com/my-account, and I will not dispute recurring charges as long as they align with this contract.
I guarantee I am the legal cardholder or authorized signer on this account.
